The cramping sensation comes from two things:
- Follicular fluid released from the ruptured follicle irritates the lining of the abdominal cavity (the peritoneum), which is very sensitive.
- Muscular contractions of the fallopian tube as it sweeps the egg toward the uterus.
What Does Ovulation Pain Feel Like?
The experience varies dramatically between women. It may feel like:
- A quick, sharp stab lasting seconds
- A dull, persistent ache on one side that lasts a few hours
- A cramp that switches sides from cycle to cycle (because ovulation alternates between ovaries)
Location is the key clue - ovulation pain is almost always felt on one side only, in the lower abdomen.
Is Ovulation Pain Normal?
Yes. Research suggests around 20% of women experience some degree of Mittelschmerz regularly. It is not harmful and is a completely normal part of the ovulatory process.
When Should You See a Doctor?
Ovulation pain becomes a concern if:
- It is severe and lasts more than 2-3 days
- It comes with fever, vomiting, or unusual discharge
- The pain is accompanied by heavy mid-cycle bleeding
These symptoms can indicate conditions like ovarian cysts, endometriosis, or appendicitis, which require medical evaluation.
