Ovulation Cramps: What They Are and Why You Get Them

If you have ever felt a sudden, sharp twinge of pain on one side of your lower abdomen around the middle of your cycle - you may have experienced ovulation cramps, medically known as Mittelschmerz (German for "middle pain").

What Exactly Is Happening?

Each month, one of your ovaries prepares a dominant follicle containing an egg. Around day 14 of a 28-day cycle, a surge of Luteinising Hormone (LH) triggers the follicle to rupture and release the egg - this is ovulation.

The cramping sensation comes from two things:

  1. Follicular fluid released from the ruptured follicle irritates the lining of the abdominal cavity (the peritoneum), which is very sensitive.
  2. Muscular contractions of the fallopian tube as it sweeps the egg toward the uterus.

What Does Ovulation Pain Feel Like?

The experience varies dramatically between women. It may feel like:

  • A quick, sharp stab lasting seconds
  • A dull, persistent ache on one side that lasts a few hours
  • A cramp that switches sides from cycle to cycle (because ovulation alternates between ovaries)

Location is the key clue - ovulation pain is almost always felt on one side only, in the lower abdomen.

Is Ovulation Pain Normal?

Yes. Research suggests around 20% of women experience some degree of Mittelschmerz regularly. It is not harmful and is a completely normal part of the ovulatory process.

When Should You See a Doctor?

Ovulation pain becomes a concern if:

  • It is severe and lasts more than 2-3 days
  • It comes with fever, vomiting, or unusual discharge
  • The pain is accompanied by heavy mid-cycle bleeding

These symptoms can indicate conditions like ovarian cysts, endometriosis, or appendicitis, which require medical evaluation.
